If you would like for me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), please download the Medical Release form below, fill it out, save it to your desktop and then attach it to an email to me by clicking the adjacent email button.  You may either email a copy to your other provider or make a hard copy to hand deliver or send by mail.
